Help shape our community’s five-year roadmap that guides how government partners, nonprofits, cities, and the County work together to prevent and end homelessness in Santa Clara County. The Santa Clara County Continuum of Care has been working with people with lived experience, service providers, cities, the County, and other key partners to draft the next iteration of the Community Plan to End Homelessness.
